How to Apply for an Ohio Provisional Graduate License
The
Provisional Graduate License is for an applicant who has graduated
from an AVMA approved college and is waiting to take the North American
Veterinary Licensing Exam (NAVLE) for the first time or if the applicant
has failed the NAVLE and is waiting to take it again.
The
Ohio Board may issue a Provisional Graduate License if the following
requirements are met:
- Complete
the State of Ohio application for a Provisional Graduate License.
The application needs to have proper notarization and a recent
photograph attached.
- The Ohio
Board must receive directly from the college/university an official
copy of your transcript with the appropriate college seal and
a photocopy of your diploma.
- The Ohio
Board must receive a letter of recommendation from a licensed
veterinarian who will be directly supervising and responsible
for you.
- You must
complete both an Ohio criminal background check and an FBI criminal
background check, conducted by the Bureau of Criminal Identification
and Investigation (BCI&I).

- Submit the
required $100.00 fee in the form of a cashier's check or money
order (personal checks will not be accepted).
A
Provisional Graduate License is valid for six months and is not
renewable.
