How to Apply for an Ohio Veterinary Medical License:


    1. Submit a Completed Application with proper NOTARIZATION and PHOTOGRAPH attached .

    2. Have a TRANSCRIPT of your college coursework with appropriate college seal, DELIVERED DIRECTLY FROM THE COLLEGE/UNIVERSITY TO THE OHIO BOARD.

    3. Submit a photostatic copy of your veterinary college DIPLOMA. You may just make a xerox copy.

    4. Have three (3) letters of PERSONAL REFERENCE submitted to the Board. These letters must comment on your "moral character" from any adult who has known you for at least one year and is not a member of your family.

    5. If you are soon to graduate, or are a recent graduate (same year as application) and the final transcript and copy of your diploma are not yet available, you may have the DEAN of your college submit a letter attesting to your pending/recent graduation, giving the EXACT DATE OF GRADUATION. The official transcript and copy of your diploma must be received by the Board no later than six weeks after graduation.

    6. If you are now, or ever have been licensed in another State or Province, you must have the veterinary board of each and every State of Province forward directly to the Ohio Board a certification of license.

    7. You must have passed the NBE and CCT examinations with a 75.0, or above, converted score on each, OR have passed the North American Veterinary Licensing Examination (NAVLE) with a 75.0 or above converted score. These scores must be transferred to the Ohio Board by the American Association of Veterinary State Boards', Veterinary Information Verifying Agency (VIVA), P.O. Box 801614, Kansas City, MO 64180-1614, phone toll free (877) 698-8482 or (816) 931-1504. Scores must be recieved before an application can be reviewed by the Board.

    8. Submit the required fee of:
    On or after the first day of March in even numbered years - $375.00
    On or after the first day of March in odd numbered years - $250.00

    9. IF YOU ARE A GRADUATE OF A FOREIGN SCHOOL: Ohio requires that all foreign graduates complete the AVMA ECFVG program in order to be eligible for licensure in Ohio. A copy of your certificate of completion must accompany your application. A letter from the AVMA ECFVG committee must be forwarded directly to the Ohio Board.

    10. Submit to a criminal records check . The criminal records check must be completed by the bureau of criminal identification and investigation in which the results indicate that the individual has not been convicted of, pleaded guilty to, or had a judicial finding of guilty for any violation set forth in section 4741.22 of the Revised Code.

    (a) An applicant requesting a criminal records check shall provide the bureau of criminal identification and investigation with the applicant’s name and address and with the Ohio veterinary medical licensing board name and address (77 So. High St., 16th Floor, Columbus, OH 43215)

    (b) The applicant shall ask the superintendent of the bureau of criminal identification and investigation in the request to obtain from the federal bureau of investigation any information it has pertaining to the applicant.

    Ohio Veterinary Medical Licensing Board Criminal Records Check

Licensure by Reciprocity:
The Board may issue a license without the examination requirement to an applicant from another state or country IF all of the following are met:

The applicant:

  1. Graduated from an AVMA approved college OR completes the AVMA ECFVG program;
  2. Holds a valid, current, unencumbered license from another state. Letters of Good Standing from the states of licensure should be sent directly to the Ohio Board.
  3. Must notify the board if they are under investigation for an act which would constitute a violation of Chapter 4741. of the Ohio Revised Code.
  4. Must submit to a criminal background check.
  5. Submit a fee: on or after March 1 of even numbered years of $425, of odd numbered years of $300.

Limited License:
How to Apply for a Limited License

1. Submit a completed application with a photograph.

2. Submit a letter from the accredited veterinary college, program of veterinary technology or governmental laboratory addressed to the Ohio Veterinary Medical Licensing Board indicating employment/internship status.

3. Submit the required fee:

  • To practice veterinary medicine as an instructor, researcher, or diagnostician: $155.00
  • To practice veterinary medicine as an intern, resident in a veterinary specialty, or graduate student: $35.00

Please note: A transcript or other letters of reference are not required for a Limited License.

Temporary Permits:
The Board may issue a temporary permit without the examination requirement to a veterinarian holding a license not revoked, suspended, or expired in another state. The applicant must submit:

  1. An application through an Ohio licensed veterinarian;
  2. The letters of good standing from the states of licensure and should be sent directly to the Ohio Board; and
  3. The fee is $100.00.

The temporary permit authorizes the permit holder to act as a veterinary consultant or to provide veterinary medical services in Ohio for a specific veterinary practice or animal for a period of six months. A temporary permit can be extended for one period of six months.

Provisional Graduate License:
The Provisional license is for the applicant who has graduated from an AVMA approved college and is waiting to take the national exam for licensure for the first time. The applicant shall:

  1. Complete the application form;
  2. Submit a letter of recommendation from a licensed veterinarian who will be directly supervising and responsible for the applicant.
  3. Submit a fee of $35.00.

All fees must be in the form of a cashier's check or money order! Personal checks are not acceptable for initial licensure.
